I am putting here one of my work.
Type-2 Diabetes Mellitus (T2DM) is the identified health problem in this paper in which the body is either not able to produce sufficient insulin or resist insulin. The key symptoms are blurry vision, thirstiness, hunger, frequent urination, and extreme fatigue (Lehmann, 2021). The paper is presenting a primary healthcare initiative for the chosen target population for increasing awareness and better management of type-2 diabetes mellitus. It is necessary to ensure better healthcare results, lower rate of hospitalizations, reduced number of death rate, and better association between primary healthcare system and the chosen target population.
In 2018-19, 13% of ATSI people had diabetes or hyperglycemia being 2.8times higher as compared to non-Indigenous people (ABS, 2019a). Higher prevalence of this disease is seen in aged people of 45years and above in 2017-2018. In the same year, the prevalence rate was highest amongst people of age 75years and above a 18.6% level (AIHW, 2021). All this is increasing because of lack of knowledge and awareness about the necessity of healthy lifestyle and weight management in preventing and managing type-2 diabetes management amongst the aboriginal people and specifically the old age people who lack English language proficiency, have poor past experiences with primary healthcare system, and do not access the healthcare services on time and are not able to make proper changes in their lifestyle for better outcomes. Aboriginal or Indigenous people living in very remote areas have higher prevalence at 9.3 per 1000 as compared to lowest rate in inner regional areas at 4.2 per 1000 and this makes diabetes a lot more concern in this population (AIHW, 2021).
