As a part of nursing assessment, it is important for the nurse to obtain a detailed history

comprising of demographic details, medical history, surgical history, medications, previous hospitalisations and any previous treatment history. Besides this, the nurse needs to perform a comprehensive physical assessment in order to determine the signs and symptoms indicative of chronic heart failure. It is important for the nurse to consider whether the signs and symptoms occurring are in the context of cardiac damage (Alpert et al., 2017). In this case, Mrs Yindi complains of bilateral pedal oedema, shortness of breath, fatigue and nocturia. The nurse must also assess other symptoms of heart failure including orthopnoea, paroxysmal nocturnal dyspnoea, reduced exercise tolerance, chronic cough and chest discomfort (Arrigo et al., 2016). This is in line with NMBA standard 4; comprehensively conducts assessment

where the nurse utilises assessment that are holistic as well as culturally appropriate for the

client (Nursing and Midwifery Board of Australia, 2016).


Besides this, the nurse must also assess for symptoms of mood disturbances. This is because, depression and anxiety have been linked to heart failure and is often diagnosed collateral in many patients with chronic heart failure. The nurse need to assess psychological needs associated with chronic heart failure for Mrs Yindi (Obiegło et al., 2016). In addition to this, it is extremely important for the nurse to obtain frequent observations of vital signs including BP, heart rate, respiratory rate, oxygen saturation and temperature. The nurse must be able to correlate the vital signs with signs and symptoms (Brugts et al., 2018).
